As well, you will need to have a membership from your local association). PDF Version Member Association Forms ~ Association Secretaries, please submit to the CTCPA officeShow Sanctioning Application - to be filled out by each association Show Advertising Form - to be filled out by each show operator so the CTCPA can promote your show. 2008 Affiliate Membership Payment Form - This form is to be used by Association Secretaries when submitting affiliate membership forms. 2008 Show Participation and Levy Calculation Form - To be filled out by each show operator or Association after each CTCPA Sanctioned Show and submitted with participation fees not more than 21 days after a show. If payment is not submitted, the show will not qualify as a CTCPA sanctioned show. | ||
